The City of Mansfield has opened a clinic where you may receive a COVID-19 vaccine at no cost. Those interested in receiving a vaccine MUST register for an appointment at Be prepared to enter your name, date of birth, address, mobile phone number and other personal information about yourself.

You do NOT have to be a Mansfield resident to take advantage of the clinic. Proof of health insurance is also not required. If you have already received at least one dose of a COVID-19 vaccine, please indicate that on the registration form

Check the registration form for information about the location of the clinic and for available dates and times to register for an appointment. For other questions or concerns, please call 817-728-3999 or email

As of Thursday, Nov. 4, 2021, the clinic is NOT offering the Pfizer-BioNTech pediatric COVID-19 vaccine, which the Centers for Disease Control and Prevention (CDC) has recommended for children aged 5 to 11. We are waiting for a shipment of this vaccine to arrive and we will advise when we are able to offer it.

If you are homebound and require assistance, please call Kristin Hart with the Mansfield Fire Department at 817-276-4791.

All personal information will be kept confidential and will not be distributed in any way. Personal data is collected only for the purpose of mandatory vaccine reporting per CDC and state requirements, and for the purpose of reminding recipients of their follow-up booster vaccine.
